Time Cards

Either client sign-in logs or Premier time cards are used to record and pay work time. Sign-in logs are generally used when the client is using a fairly high volume of Premier’s temp staff. If the client does not require the use of a sign-in log then a Premier time card is used. Daily time cards are used for per diem or day by day assignments. Weekly time cards are completed for contracted and/or routinely scheduled assignments. It is your responsibility to acquire authorizing signatures for your time worked regardless if you fill out a time card or a sign in log. Time that is not authorized will not be paid.  In all cases time cards that are submitted more than 45 days past the date of work will not be processed.

When signing a sign-in log you should also complete a Premier time card for your personal records. If you fill out a time-card and log, both records must have identical sign-in, sign-out and lunch data. Remember that when a sign–in log is required, the log is the document being used to record and pay work time, not the PREMIER time card. 

Whether you are using a sign-in log or a PREMIER time card, you must record the facility name, the work date, the start time, recorded lunch break times or the approval of the supervisor authorizing no-lunch taken, the end time, your signature and the signature of the manager approving your overall time card. Note that when No Lunch is indicated on your time card, you must have a separate approval noted in the area where No Lunch was written. This approval is in addition to the approval on the “Delegate signature line” of the time card.

Complete your Premier time card using dark ink and write clearly. A copy of your time card should be left with the facility and a copy should be faxed to PREMIER. Fax your time card as soon as possible.  A separate time card is generally required for each shift worked.  Please follow the instructions for faxing your time card found on the top of the PREMIER time card.  Time cards can be faxed seven days a week, 24 hours a day. The sooner you fax the timecard to us the more time we have to verify it or request another one if the fax transmission was disrupted We do not recommend mailing your timecard as the mail can be delayed and we have no way of tracking missing mail. If you must mail your time card, allow enough time for it to arrive in our office.  If we have not received your time card by noon on Monday we may contact you requesting you to fax it to us. Time cards received after the noon deadline on Monday will be processed with the following weeks’ payroll.
